• Wednesday, July 29th from 7:30 am – 11:30 am

            A computer will be available for your use.

• Immunization Record (signed by a nurse) if not uploaded during registration.

• PK -12 Permissions and Notifications
• Forms
  • Preschool Age Child Health Form
  • PS Dental Screening (*optional)
  • PS Vision Screening (*optional)
