Kindergarten

• • Certificate of Immunization completed by a healthcare provider, or Medical Immunization Exemption Form signed by a healthcare provider, or Religious Immunization Exemption Form to be notarized

  • Physical Form completed by a healthcare provider

  • Vision Screening Form - No earlier than 1 year prior and no later than 6 months after enrollment.
• • Dental Screening - No earlier than age 3 and no later than 4 months after enrollment.

   7th Grade

• • Certificate of Immunization completed by a healthcare provider to include boosters of the Tdap and Meningitis vaccines. 

  • Iowa law requires boosters before the first day of school.

   9th Grade

• • Dental Screening Certificate
