Kindergarten-12th Grade students need a record of completed immunizations.
Immunizations Required:

At least four Dtap and IPV (polio), with one received after age 4 yr. old.

Two MMR and Varicella (or a record of natural disease).

Three doses of Hepatitis B.

Kindergarten boosters: Dtap/IPV, and MMRV combination vaccines.
Further requirements:

Students entering 7th grade:

1 dose of meningococcal vaccine (MCV4 ) on or after age of 10. A second shot/ booster dose will be needed at age 16.
